The following muster roll was transcribed from the original document and reflects information exactly as it was written. Names not legible due to handwriting were resolved using the Company "D" Roster. Also, the creation date of this document is March 1, 1863. It should be noted that several dates listed in the "REMARKS" column reference dates later than the creation date. It should be assumed that the writer of this document incorrectly specified 1863 when he intended to write 1862. This assumption is supported by the reference to the battle of Chickasaw Bluffs on December 29th, 1863, when, in fact, this battle occurred on that date in 1862. Information listed which is suspected in error or for which transcription was difficult is shown in
red.

Note that just eight weeks prior to this muster roll, the 16th OVI was engaged in the Battle of Chickasaw Bluffs, near Vicksburg, Mississippi, and suffered heavy losses. The commander of Company D, Capt. Milton Mills, was taken prisoner and did not return to the Company until his exchange in May, 1863.

I certify, on honor, that this Muster Roll is made out in the manner required by the printed notes; that it exhibits the true state of Captain Milton Mills Company ( D ) of the Sixteenth Regiment of Ohio Vols Infantry for the period herein mentioned; that the "Remarks" set opposite the name of each officer and soldier are accurate and just; and that the "Recapitulation" exhibits in every particular the true state of the company, as required by Regulations and the Rules and Articles of War.
